Reservation Form - Serenade Punta Cana Group Trip

Thank you for your interest in joining our group trip to the beautiful Serenade Punta Cana Beach and Spa Resort from January 7th to 14th, 2026. 

Please complete one form per traveler/couple. Once received, I’ll follow up to confirm your reservation, passenger details, and provide secure payment instructions.

Important Notes:

• Please complete one form per room
• No payment will be collected at this stage

Payment Schedule:

• Non-refundable deposit of $200 per person is due at time by July 25th, 2025 to secure your spot
• Payment in full due by October 24th, 2025

Cancellation

• Deposits are non-refundable
• 59-22 days prior to departure: 50% of the total package cost per passenger including taxes, fees, and surcharges
• 21 days or less prior to departure: 100% of the total package cost per passenger including taxes, fees, and surcharges
