This form will provide an overview of you and your travel companions as a whole and what kind of vacation you want to plan.

The form provides the building blocks necessary to build your travel package. Please answer everything as truthfully and accurately as possible. If I have any questions, I will reach out to you immediately and possibly schedule a telephone or zoom meeting for clarifications

There is no fee associated with the submission of this form. All my travel services are free to my clients, as I am paid exclusively by travel companies after you complete your trip.

One of the purposes of this form is to indicate your willingness to work with a travel agent in booking your travel needs. If your goal is to book on your own in the end, kindly reconsider submission of this form.
