This form will provide an overview of you and your travel companions as a whole and what you are looking for in a cruise vacation.

The form provides the building blocks necessary to build your travel package. Please answer everything as truthfully and accurately as possible. If I have any questions, I will reach out to you immediately and possibly schedule a telephone or zoom meeting for any clarifications.

The information gathered here will never be used outside of planning and booking your vacation, or sold or distributed to any outside agency.

There are no fees associated with the submission of this form. My travel services are free to my clients, as I am paid exclusively by travel companies once you complete your trip. 

Before filling out this form, please be certain you intend to book a cruise vacation with the assistance of a travel agent. If your intention is to book on your own in the end, please reconsider submission of this form.
