04. REPEAT CLIENT PROFILE FORM

If we are making an air reservation, the

passenger name(s) MUST match the passport

exactly in order to board the plane. If you are

going through a name change, please consult

your agent prior to submission.

NOTE: If you don’t have a passport as of yet,

please include the information that will match

your passport application and omit any details

that you currently do not have available. You will

be given another opportunity 90 days prior to

travel to provide these details.
