Cancellation Request Form

I have agreed to all Terms & Conditions and the Cancellation Policy of Tim Cruise Travel Agency at the time of purchase. I have previously been advised to obtain travel insurance to protect my trip. The cancellation date will be considered the date I sign and submit this signed form.

By completing and submitting this form, I understand that I am authorizing the cancellation of the reservation(s) referred to above and may incur penalties as a result of this cancellation. I understand that if I have travel insurance, it is my responsibility to obtain and submit all relevant information for my claim to the travel insurance provider.
