Form Current Members & Established Clients of Exemplary Travel Advisors
If you already have your information on file with one of the advisors of Exemplary Travel Advisors, simply select their name below and we will import your information to the trip. You do not need to fill out the personal information again aside from your name and email. We would just need your travel preferences.
Traveler 1
If you are not a current travel club member, a separate invoice will be sent to you to pay for your travel club membership. The rate is currently $97 per year.
Conditions of Traveling with the Group
Please note, as a condition of traveling with us, you must book the group hotel through our agency in order to participate in the group events. You are not required to book any of the dinners, activities, or flights through our agency (though we would love to have you there).
Hotel Loyalty Information
For World of Hyatt members, please list your loyalty information in this form so that we can make sure to book rates that will get you the points you want to earn. World of Hyatt rates may or may not not include all of the Signature amenities listed but we will let you know in advance if that is the case.
Travel Club Membership
Membership are per household living at the same address. If you are sharing a room with someone outside of your immediate household, we will charge the membership fee separately to each of you.
