By depositing this vacation, you have agreed to the terms and conditions of

PASSPORTS & VISA
If you are traveling outside of the United States and her territories, Passport Books are required for all travelers (including infants & children) and must be valid for 6 months beyond the date of travel.
Depending on the country visited, you may also need a VISA and/or tourist card along with your passport. Up to date travel and passport information is available at http://travel.state.gov/foreignreqs.html.
Failure to have proper documentation will result in denied boarding with no refund.

WARNING TO ALL CLIENTS TRAVELING TO CANADA- effective 2009, if you have any felony convictions, including but not limited to DUI/DWI, shoplifting, assault, and more, you may not be able to enter Canada without obtaining a special waiver, well in advance of travel. HONEYMOON & ANNIVERSARY TRAVELERS:
Please bring a wedding invitation or a copy of your marriage certificate to qualify for any complimentary honeymoon/anniversary package (photo copy is recommended). Failure to have such proof of marriage may result in the resort denying these complimentary privileges (if applicable).
TRAVELING WITH CHILDREN:
Children under 18 years of age not traveling with both parents or legal guardian(s) may need to present a notarized letter from the absent parent/guardian granting permission to travel outside the United States and provide emergency medical care if necessary. CRUISE PASSENGERS:
PRICE INCLUDES: Cruise pricing is per person based on two people in the cabin and includes your stateroom, most meals aboard ship including breakfast, lunch and dinner in the dining room or buffet (except specialty restaurants), and most onboard entertainment.
PRICE DOES *NOT* INCLUDE: Unless otherwise noted - transportation to or from the port, parking, travel insurance, alcoholic beverages and specialty drinks, specialty restaurants, gratuities; or any items of a personal nature such as spa treatments, shore excursions, photographs or medical services.
*** PREGNANCY: You must notify My Vacation Lady immediately if a traveling passenger will be pregnant. Policy varies by cruise line but most will not accept guests who will have entered their 24th week of pregnancy by the beginning of or at any time during the cruise.
FLYING TO YOUR CRUISE PORT: It is highly recommended that you fly to your cruise port the day prior to the cruise departure. If you miss the ship due to flight delays, you will need to arrange for transportation to the next port of call at your own expense. Please contact My Vacation Lady before booking the flights so that we can advise you of the earliest return flight to book.
MEDICAL QUESTIONS/TRAVEL IMMUNIZATIONS/PRE ENTRY COVID TESTING
It is the traveler’s responsibility to ensure that they are properly immunized for travel to their destinations. For the most up to date Covid testing protocol, please check with the tourism board of your destination. Please check on the CDC’s website for the most up to date travel warnings and alerts. http://wwwnc.cdc.gov/travel/default.aspx.
SPECIAL REQUESTS
Special requests such as room locations, bedding preferences, airline seat assignments, etc. cannot be guaranteed by My Vacation Lady or Travel Suppliers. While we will make those special requests on your behalf, My Vacation Lady cannot be held responsible if the requests are not fulfilled by the hotel, airline, etc.
CHANGES TO RESERVATIONS
Any changes made to your existing reservations must be in writing and are subject to the current rates, promotions, and terms at the time of change.
FINAL PAYMENT
Your final payment MUST be received NO LATER THAN the date listed on this invoice (usually 60-90 days prior to departure) and is not automatically charged. Please note your calendar of this final payment date and be prepared to pay the remaining balance at that time
* My Vacation Lady is not responsible for supplier cancellations due to payments made after this date. Late fees will apply.
LATE FEES
All reservations are subject to cancellation and hotel penalties as described above if final payment is not received by the due date. Final payments not received prior to the due date are subject to a $25 per person late fee.
CANCELLATION PENALTIES: Once your reservation is deposited, it is subject to a minimum of a $100 per person nonrefundable agency refund cancellation fee in addition to any supplier penalties assessed.
* Regarding Airline tickets: most airline tickets are completely non- refundable and subject to individual airline penalties/fees (see insurance below).
TRAVEL INSURANCE
Travel Insurance is strongly recommended and will cover the costs of trip cancellation for a covered reason or unexpected travel delay expenses. Travel insurance also provides coverage for baggage loss or delay and most importantly coverage for medical emergencies and evacuation which may or may not be covered by US medical policies when traveling outside of the United States.
AIRLINE SCHEDULE CHANGES & CANCELLATIONS
Airlines schedules can and do change and this is beyond My Vacation Lady’s control. If this occurs, we will do our best to assist you with finding the best possible alternate option(s). We will notify you of major changes. Please Note: there may be additional costs to re-book a new flight should the airline cancel or make a major change to your original flight itinerary, that you will not accept. My Vacation Lady is NOT responsible for any additional costs or fees imposed by the airlines.
RE-CONFIRM YOUR FLIGHTS
It is the responsibility of the passenger to check in online to check for last minute airline flight time changes within 24 hours of departure. My Vacation Lady is not responsible for changes that may occur within 24 hours of original flight departure time.
AIRPORT ARRIVAL
You must arrive at the airport at least 2 hours before your domestic flight or 3 hours before your international flight due to increased security screening. Please allow additional time during holidays and other peak departure times.
**** You must be checked in at the airport no later than 2 hours before your scheduled departure time and at the gate and ready to board 45 minutes before departure. This minimum requirement applies to all customers checking-in, with or without baggage. Customers who fail to complete the check-in procedures by these deadlines may be denied boarding and change fees and additional airfare will be charged to continue travel.
BAGGAGE FEES
Due to continual changes in airline baggage policies it is suggested that you inquire with your airline's website for up-to-date fees & information. My Vacation Lady is NOT responsible for additional fees incurred for baggage or seating. BAGGAGE FEES ARE NOT INCLUDED IN THE FLIGHT COSTS
HOTEL CHECK IN
Check-in time at most hotels is 3:00pm (local time). If you arrive prior to 3:00pm your room may not be ready. The hotel will store your luggage in a secure location on property so you may start enjoying your vacation without delay.
DRESS CODE: ** MEN ** - Please be advised that some resort restaurants may require long pants, collared shirt, & closed-toe shoes. Please pack accordingly.
TRAVEL DOCUMENTS
My Vacation Lady will supply travel vouchers prior to departure. It is the traveler’s responsibility to READ those vouchers and contact us with any questions PRIOR to departure.
